Forms

Teachers and students can download the following forms to assist with project development and management:

• Student Profile and Participation Agreement-to collect basic student information and to document student and parent agreement
• TPSP Student Form-to collect more detailed student information, including number of years the student has received gifted/talented services
• TPSP Teacher Form-to collect more detailed information about the teacher, which may be helpful if schools and/or districts are performing program evaluations
• Topic Submission and Approval-to provide information about the desired topic of exploration
• Weekly Planner-to record week's goals and daily plans
• Research Plan/Proposal-to guide students' research plan and identify possible resources
• Primary Resource Process Sheet-to document learning from primary resources
• Secondary Resource Sheet-to document learning from secondary resources
• Periodic Progress Check-to review the student's progress throughout the course of the project
